I'm no doctor but when u put together the recent weight gain and what could be a rash on her face she is trying to cover up, my first thought is lupus. Prednisone will make you gain weight or appear to be swollen, and some people with lupus get what is called a Mylar rash(also called a butterfly rash) which is red and spreads across cheeks and nose. The rash isn't permanent and appears during a flare up. BUT it could just be shitty procedure gone wrong

Can't tell if it's plastic surgery but definitely bad lighting, bad hair color, bad make-up job. They do that because making you squirm gives them a feeling of power. They won't stop doing it either, nor will they ever consider your feelings. They enjoy bullying when they get to be the bullies. (not so much when they are the victims, but as long as you're around, they'll never be lowest on the totem pole).
Art school story: There was this little group of 3 in my school. Two guys, one girl. Girl was not the girlfriend of either guy. They always stuck together. I would overhear them talking. Guys would talk normally to each other, a fair bit of joking around but nothing mean = conversation of equals. Girl would say something. Immediately both guys would make fun of her and/or invalidate anything she had to say -- in a fakey jokey manner that would be easy to dismiss as kidding if you didn't perceive the pattern. Whatever she did wasn't as good as what the guys did, whatever she said was dumb, whatever she liked was stupid. And she always responded to them as if this were a normal way of being treated.
She got into an accident and broke her back, and never came back to class. Her so-called friends barely mentioned her after that and showed no sympathy for what happened to her, though they had lots of dudely compassion for other guys.
It would be easy to dismiss those guys as just an isolated case of a couple of assholes - but such cases aren't isolated. They're all over the place. They're the norm. Men don't relate to women because deep down, they don't believe women are fully human beings.
