I don't think her arms are actually muscular, but the bulging veins on too thin arms make them look that way. Many alcoholics who favor hard liquor actually don't eat much when they are bingeing, so she is probably just "skinny" from the alcohol.
Which explains the "just rolled in pigshit" look she has about her.
